Your appointment time is reserved specifically for you. If you need to cancel or reschedule your appointment, we request that you do so at least 48 hours before your scheduled appointment time. At this time, Cloud 9 Acupuncture does not submit claims to insurance companies on behalf of clients. However, we are happy to provide you with a "SUPERBILL" with all the necessary medical coding and information required for reimbursement. Please check with your health insurance company to determine if you have coverage and if prior authorization is required by your policy. We encourage you to use your Health Savings Account (HSA) or Flex Spend cards to pay for your treatment if that is an option for you.
Gift certificates are not redeemable for cash and cannot be returned for a cash refund. Gift certificates, once entered, will be automatically applied to your next session. If a session exceeds the amount of the gift certificate, the balance of the treatment must be paid with a credit card or cash at the time of service. Any unused balance will be placed as a credit on your account. We are not responsible for lost of stolen gift certificates.
